Apply for a Severely Disabled Person's ID (Schwerbehindertenausweis)

• In the assessment procedure you were granted a Grad der Behinderung (degree of disability, GdB) of 50 or more service.berlin.de/dienstleistung/326173/ • You are applying for a severely disabled person's pass (Schwerbehindertenausweis) for the first time, or your existing pass is damaged, expired, about to expire, or there has been a change (name, degree of disability, markers/Merkzeichen) Notes: • You can apply for the renewal of your Schwerbehindertenausweis 4-6 weeks before its expiry date. • No new application for assessment of disability needs to be submitted for renewal of the pass card. • Please notify the Versorgungsamt (pension/welfare office) of any changes to your name or address (see "Further information"). • If you have lost your pass, you can apply for a replacement to be issued (see "Further information"). • Residence in Berlin You live in Berlin and are registered here. • If you have moved to Berlin from another federal state, your file must be transferred to the Versorgungsamt Berlin.

• Current photo (from age 10) To apply for the issuance of a Schwerbehindertenausweis (severe disability ID card), send the Versorgungsamt (pension office/welfare office) a photo/passport photo of yourself (it does not need to be biometric). You can do this online, by email (in JPG format), by post, or in person. • For foreign citizens from non-EU states: valid Aufenthaltstitel (residence permit) in Germany (copy) • If you are a citizen of a European Union (EU) state, a copy of your identity document is generally sufficient. • If you are a citizen of another state, we require a copy of your residence permit, for example your Aufenthaltserlaubnis (residence permit) or your Aufenthaltsgestattung (permission to reside pending asylum procedure). • If represented: power of attorney If another person is to obtain the Schwerbehindertenausweis on your behalf, that person must submit a written power of attorney.
