• Application for issuance of a Unternehmenskarte (company card) The fully completed application form must be presented at the in-person appointment. Applications submitted by post cannot be processed. • Identity card or passport of the business owner or the person authorized to represent them (copy) • If applicable, additionally: power of attorney for the representation please bring a power of attorney for the person attending, if applicable • If applicable, additionally: identity card or passport of the person responsible for vehicle deployment (copy) • Business registration (Gewerbeanmeldung) or extract from the commercial register (Handelsregister) (copy)

The issuing office for Kontrollgerätkarten (memory cards for digital tachographs) in Berlin is the Landesamt für Bürger- und Ordnungsangelegenheiten (State Office for Citizens' and Regulatory Affairs, LABO). Applications can only be submitted at the Puttkamerstraße service building.
