• Application for Grundsicherung (basic income support) in old age and in case of reduced earning capacity under SGB XII (Book Twelve of the Social Code) Please send the completed and signed application, together with copies of the required documents, by post to the responsible Amt für Soziales (social welfare office) of your home district, or submit the application in person. • Valid personal identity documents if applicable, Meldebestätigung (registration confirmation) • Proof of permanent, full reduction in earning capacity • Proof of income • Proof of assets for example, for capital-forming insurance policies (life insurance, Bausparversicherung [building society savings contract], Riester pension contracts, Sterbegeldversicherung [death benefit insurance], funeral provision plans and similar), savings accounts, land, real estate, valuables, motor vehicles • Bank statements • Tenancy agreement if applicable, notice of rent change • Proof of health and long-term care insurance • The scope of documents required, in particular proof of income and assets, depends on the specifics of the individual case.
