If your child is to attend ergänzende Förderung und Betreuung (eFöB, supplementary support and care) offerings at school, you must submit an application for supplementary support and care to the Jugendamt (youth welfare office) responsible for you at least three months before the desired start of care. Sole custody can be proven by a Negativbescheinigung (a certificate confirming that no other person holds joint custody) or by a court order. • Copy of the foster care agreement (Pflegevertrag), if the child is a foster child • Meldebescheinigung (registration certificate) where an Auskunftssperre (information blocking notice) is in place • For children in the upper level (Oberstufe) or final level (Abschlussstufe) at a school with the special educational needs focus "Geistige Entwicklung" (intellectual development), or in grades 7 to 10 at a commissioned school (Auftragsschule) with the special educational needs focus "Autismus" (autism): documents for the needs assessment Depending on your employment or training situation, the following documents are required from both parents, provided the child is cared for by both parents: • a current confirmation from your employer of your working hours, since when you have been employed there and to what extent, and a confirmation of any parental leave (Elternzeit) taken (no older than 12 weeks); • certificate of training, coursework, or enrollment (Ausbildungs-/Schulungs- oder Studienbescheinigung); • proof of your self-employed activity, for example a trade registration (Gewerbeanmeldung) or a contribution notice from the Künstlersozialkasse (artists' social insurance fund) • From the 4th grade onward: declaration for determining the contribution toward the cost of day care (income statement) for the period 01.01. to 31.12. of the last calendar year (as a copy) • Income tax assessment notice (Einkommensteuerbescheid) for the last calendar year. If the notice is not yet available, please submit provisional income statements for the last calendar year; a self-assessment of gross income is possible.
