Severe disability – Report a change

• Assessment procedure under the Schwerbehindertenrecht (law on severe disability)

• Änderungsmitteilung für das Schwerbehinderten-Feststellungs-Verfahren [notification of change for the severe disability determination procedure] (under "Formulare" [Forms]) • A move within Berlin can be reported by phone. • Moving to Berlin or moving away from Berlin must be reported in writing. • A change of name can be made in writing or in person. • Notification of death must be made in writing. • if applicable, proof of the change of address/name Suitable documents are the Personalausweis (identity card), passport, or other civil status documents such as a name change certificate or marriage certificate • if applicable, death certificate • The Bescheid (notice) or the Schwerbehindertenausweis (severely disabled person's ID) should be kept for some time to settle related matters. • The ongoing procedure ends upon the death of the applicant. No further Bescheid will be issued. If heirs require a notice under the law governing severely disabled persons, it is requested from the responsible Versorgungsamt (welfare office) by the competent Finanzamt (tax office) by way of administrative assistance.
