• The last professional activity must have been performed in Berlin.

• Proof of activity in the State of Berlin (Certificate from the competent professional chamber (Berufekammer) regarding the data held there.) • Change-of-name certificate (if applicable) • Doctoral degree certificate (if available) • Further training title (e.g. Facharzt/specialist physician), if this is to be included in the certificate • Certificate of completed training (e.g. certificate of the Ärztliche Prüfung/state medical examination) (only if the training was not completed in Berlin and/or the Approbation (professional licence) or permission to use the professional title was not issued in Berlin) • Approbationsurkunde (certificate of professional licence) or certificate of permission to use a professional title (only if the training was not completed in Berlin and/or the Approbation or permission to use the professional title was not issued in Berlin) • Important: If copies are submitted, they must be officially certified. For copies without official certification, the originals must be presented at the same time.
