Severe disability - submitting taxi receipts for the special transport service

• Authorization number (Berechtigten-Nummer) for participation in the Sonderfahrdienst (special transport service) You must be authorized to use the Berlin Sonderfahrdienst (special transport service) and have an authorization number. To do this, apply to the Versorgungsamt (welfare office) for the "T" mark (participation in the special transport service). The "T" mark is issued to people with an exceptional walking disability (mark "aG" in the Schwerbehindertenausweis (severely disabled person's ID)). • Have a receipt issued for the travel costs paid.

• Application for reimbursement of taxi receipts Please submit the application in writing. • original taxi receipts Make sure that neither the date nor the amount payable has been corrected, overwritten, or written with different pens, as otherwise they cannot be accepted and reimbursed. Receipts for taxi rides are accepted by the Versorgungsamt (welfare office) if they contain the following information: • name and address of the operator with licence number (stamp) • route (starting point and destination) • fare/tax rate • date of the trip • signature of the driver
