Severely disabled person's ID (Schwerbehindertenausweis) - Applying for the supplementary sheet (Beiblatt) with or without a valid travel stamp (Wertmarke)

• Two-coloured Schwerbehindertenausweis (severely disabled person's ID card) A valid two-coloured Schwerbehindertenausweis must be held. The ID card states the Merkzeichen (markers indicating the type of impairment). • Payment received by the Versorgungsamt (pensions office) • Once the payment has been received by the Versorgungsamt, you will be sent the supplementary sheet with the Wertmarke (valid travel token). • You will receive an information letter stating the bank account details and the payment reference to be used. • Receipt of ongoing social benefits The Wertmarke is free of charge for you if you receive one of the following benefits: • Arbeitslosengeld II (Bürgergeld, unemployment benefit II / citizen's income) • Grundsicherung (Sozialhilfe, basic income support / social assistance) or Grundsicherung im Alter und bei Erwerbsminderung (basic income support in old age and in case of reduced earning capacity) (SGB XII) • ongoing benefits under child and youth welfare services (SGB VIII) • benefits to secure subsistence under SGB XIV • as a care home resident, receive a Barbetrag (Taschengeld, personal allowance/pocket money) from the social welfare provider • as a care home resident, receive Hilfe zur Pflege (assistance with care) under Chapter 7 of SGB XII • as an asylum seeker, receive benefits under § 2 para. 1 Asylbewerberleistungsgesetz (AsylbLG, Asylum Seekers' Benefits Act)

• Application for the Wertmarke (travel token) with the marks "H", "Bl" or "Tbl" You can submit the application online or in writing. • Application for the Wertmarke (travel token) with the marks "G", "Gl" or "aG" You can submit the application only in writing. • if applicable, confirmation of receipt of ongoing social benefits If you apply for the Wertmarke with the mark "G", "Gl" or "aG" and receive benefits to secure your subsistence, you do not need to pay a personal contribution. • Have your receipt of benefits confirmed with an official seal or authority stamp by the responsible authority (e.g. JobCenter, Sozial- und Grundsicherungsamt (social welfare and basic security office), Hauptfürsorgestelle (main welfare office)). Only once this certificate has been submitted to the Versorgungsamt (pensions office) will you receive the supplementary sheet with the Wertmarke.
