• Application for social assistance (Sozialhilfe) • Valid personal identification documents Meldebestätigung (confirmation of registration) • Proof of income • Proof of assets for example for capital-forming insurance policies (life insurance, building society savings insurance, Riester pension contracts, death benefit insurance, funeral provision plans, and similar), savings accounts, land, real estate, valuables, motor vehicles • Bank statements • Tenancy agreement Rent change notice • Proof of health and long-term care insurance • Proof of the degree of visual impairment This can be: • Ophthalmological findings • Determination notice under the law on severely disabled persons (Schwerbehindertenrecht) • Severely disabled person's pass (Schwerbehindertenausweis) with the marker "Bl" (blind) • Notices of equivalent-purpose benefits This can be: • Notice on the granting of benefits under the Landespflegegeldgesetz (State Care Allowance Act) • Notice from the long-term care insurance fund (Pflegekasse) on the determination of a care grade (Pflegegrad) • Notice from the accident insurance fund (Unfallkasse) or trade association (Berufsgenossenschaft) on an accident pension • The scope of documents required, in particular proof of income and assets, depends on the specifics of the individual case.
