• You were summoned to an appointment as a witness and attended the appointment • You have not waived your witness compensation (Zeugenentschädigung) • Application must be submitted within 3 months

• Application for compensation of witnesses in civil proceedings (under "Forms") You have already received this form together with the summons to the appointment. • Payment order The "Auszahlungsauftrag" (payment order, officially: HKR 172) certifies your attendance. You will receive the original and a copy of the "Auszahlungsauftrag" after your witness testimony / after you are released from the appointment. The payment order must be signed by the court. • Proof of travel costs incurred or other expenses Please provide proof of travel costs incurred for your journey to and from the appointment by means of appropriate receipts (e.g. original tickets, booking confirmations, invoices for flight tickets, accommodation costs). • Certificate of loss of earnings Have you incurred a loss of earnings? Then please have this certified by your employer. You have already received the form for a certificate of loss of earnings together with your summons. • Proof of self-employment Are you self-employed? Then please enclose corresponding proof of your self-employment (e.g. trade license) and state your average monthly gross income from self-employed work as well as your regular working hours. • Credible evidence of freelance activity Are you working as a freelancer? Please provide credible evidence of your freelance activity by means of suitable documents. Please also state your average monthly income from freelance activity and your regular working hours.
