In that case, bring the pre-completed application form to the appointment. • For applications submitted by the host (as authorized representative): the guest's vehicle registration number, place of residence and period of stay must be stated. • For applications submitted by the guest: the host's name and place of residence must be stated. • Proof of special urgency Proof by presenting, for example: • an EU parking permit • or a special parking permit for special groups of severely disabled persons ("Gleichgestellte" [equivalent status]) • or an ID card for severely disabled persons with the code "G" • or a medical certificate • Zulassungsbescheinigung Teil 1 (vehicle registration certificate part 1) (copy) For registration numbers from Berlin and Brandenburg that were registered with a registration office outside the postal code range 10000 to 16999. • A copy of the fully unfolded front side of the Zulassungsbescheinigung Teil 1 ("Fahrzeugschein" [vehicle registration document]) is required. It must show the name and address of the registered keeper, the type of vehicle, the technically permissible total mass and the official registration number of the motor vehicle. • If applicable, proof that the vehicle has been made available to you for permanent use (under "Formulare" [Forms]) • If you, as the applicant, are not yourself the registered keeper of the vehicle, proof is required, for example, in the form of a written statement from the registered keeper that the vehicle is available to you for permanent use. • Rental car: confirmation from the car rental company that the vehicle has been made available to you for permanent use. • Personal identification document (copy) The copy of both sides of the Personalausweis (identity card) serves as proof of the registered address and of the identity of the host and the guest. If a passport is used as proof of identity, or if the actual place of residence is not entered in the Personalausweis (identity card) (e.g. in the case of a secondary residence), please provide, in addition to the copy of the identification document: • consent for the authorities to inspect the Melderegister (register of residents) • or, alternatively, a current Meldebescheinigung (registration certificate) • If applicable, proof of loss or damage A replacement Gästevignette (guest parking vignette) can only be issued if the loss or damage can be credibly demonstrated, where possible, by supporting documents and/or a written confirmation. • Note on data protection All other non-relevant data, such as height, eye colour, photograph, access number, further details about the motor vehicle, etc., may be rendered illegible in accordance with the Berliner Datenschutzgesetz (Berlin Data Protection Act).

Apply for the guest vignette (Gästevignette) in the district (Bezirk) where the parking management zone (Parkraumbewirtschaftungszone) is located and where the host is registered (Anmeldung). Exception: The Bezirksamt Mitte (Mitte district office) is responsible for the entire Flottwellstraße.
