• Direct civil servant status with the State of Berlin, or you are the recipient of a survivor's pension from the State of Berlin • You or your privately insured family members are in need of long-term care Need for long-term care within the meaning of §§ 14 and 15 of Book Eleven of the Sozialgesetzbuch (Social Code) (determination of a care level).

• Pflegekassenbescheid (long-term care insurance fund notice) from the responsible private or statutory Pflegekasse (long-term care insurance fund) • regarding the approval of a Pflegegrad (care level) and the granted care benefit • regarding the approval of a care aid (Pflegehilfsmittel), or • further documents from the Pflegekasse regarding approved benefits • proof of expenses via receipts Copies or duplicate invoices are sufficient.
