• Loss or damage to the previously valid Bewohnerparkausweis (resident parking permit) You have lost or damaged your valid resident parking permit. • You continue to be a resident of a Parkraumbewirtschaftungszone (parking management zone) • You continue to be the registered owner of a registered vehicle or are demonstrably permitted to use it permanently This also applies to rental cars. • For online applications: consent to the electronic payment procedure Pay electronically directly in the online procedure by credit card (Visa, Mastercard).

Application for Replacement Issuance of a Residents' Parking Permit You can submit the application online, alternatively in writing by post, or, in exceptional cases, in person at the district Bürgerämter (citizens' offices) of the parking zone. • For online applications: Please have all required documents and evidence ready for upload in PDF, JPG, PNG, GIF or BMP format. A single file may not exceed 5 MB. • For written applications by post: A signature on the application is mandatory! Please do not send cash or crossed cheques. You will receive a Gebührenbescheid (fee notice) with a payment request. • For applications submitted in person: A signature on the application is mandatory! In-person appointments are currently only available in exceptional cases, after prior arrangement with the district offices. Bring the pre-filled and signed application form to the appointment. • Parking permit number • Zulassungsbescheinigung Teil 1 (vehicle registration certificate part 1, in copy) A copy of the fully unfolded front side of the Zulassungsbescheinigung Teil 1 ("Fahrzeugschein") is required. It must show the name and address of the vehicle owner, the vehicle type, the technically permissible total mass, and the official registration plate of the motor vehicle. • For written or in-person applications: proof of identity (in copy) • A copy of both sides of the Personalausweis (identity card) serves as proof of the registered address. • If a passport is used as proof of identity, or if the actual place of residence is not entered on the identity card (e.g. for a secondary residence), please also submit, in addition to the copy of the identity document, consent for the authority to inspect the Melderegister (registration register) or, alternatively, a current Meldebescheinigung (registration certificate). • Proof of loss or damage The loss or damage must be substantiated as credibly as possible through evidence and/or written confirmation, e.g. through confirmation of the loss report to the police or another declaration of loss. • If represented: written power of attorney You only need a written power of attorney if you authorize someone else to apply for and collect a residents' parking permit on your behalf. • Note on data protection All other data not relevant to the application, such as height, eye colour, photograph, access number, further details about the motor vehicle, etc., may be rendered illegible in accordance with the Berlin Data Protection Act (Berliner Datenschutzgesetz).

Apply for the Bewohnerparkausweis (residents' parking permit) in the Bezirk (district) in which the Parkraumbewirtschaftungszone (parking management zone) is located. Exception: The Bezirksamt Mitte (Mitte district office) is responsible for the entire Flottwellstraße.
