Wish to Have Children - Applying for a Grant for Artificial Insemination

Eligible applicants • Same-sex female couples and different-sex married couples • Same-sex registered life partners (Lebenspartnerinnen) under the Lebenspartnerschaftsgesetz (Life Partnership Act) • Same-sex female couples and different-sex couples living in a non-marital cohabiting partnership (nichteheliche Lebensgemeinschaft) • Information on the non-marital cohabiting partnership (unmarried couples): A non-marital cohabiting partnership is a partnership between a man and a woman, or between two women, that is intended to be long-term and permanent, does not permit any other partnership alongside it, and is marked by a close personal bond. It is deemed to exist if, in the assessment of the treating doctor, the unmarried couple is living together in a stable partnership. In a non-marital cohabiting partnership between a man and a woman, the man must acknowledge paternity of the child conceived through assisted reproduction. • You are involuntarily childless • Main residence in Berlin Your joint main residence (Hauptwohnsitz) is in Berlin at the time of application and at the start of treatment. • You are carrying out the fertility treatment (Kinderwunschbehandlung) at an approved fertility clinic in Berlin. • You have not yet started treatment The start of treatment is defined as filling the first prescription at the pharmacy relating to the attempt applied for. • You are covering part of the costs incurred for the fertility treatment yourself • Counselling You have received medical as well as psychosocial counselling. • Both partners must be at least 25 years old The woman undergoing the fertility treatment must not yet have turned 40, and the man, or the woman not undergoing the fertility treatment, must not yet have turned 50, at the start of treatment. • For same-sex female couples, only the egg cells of the partner undergoing the reproductive medical treatment may be used. • Deadline for the payment application: • The payment application must be submitted by post to LAGeSo (State Office for Health and Social Affairs) within 2 months of receiving the last invoice / the last pharmacy receipt. • If you are entitled to a bonus payment or cost reimbursement from the statutory health insurance fund (Gesetzliche Krankenkasse) / private health insurance (Private Krankenversicherung) / the allowance office (Beihilfestelle), you must first assert your claims there. The letter subsequently issued then counts as the most recent proof relating to your fertility treatment for time purposes. This letter is decisive for calculating the 2-month deadline for submitting your payment application by post.

Step 1: Application/follow-up application for funding for Assisted Reproduction only possible online • The application will then be displayed as a PDF document. Please print it out and sign the application by hand (both applicants). • Afterwards, send the signed application by post to LAGeSo Berlin (Landesamt für Gesundheit und Soziales – State Office for Health and Social Affairs). The address is on the first page of the application. • Step 2: Submission of additional documents only possible online The authority has informed you by email which documents are needed for your application. • Please have all required documents and evidence ready for upload in PDF, JPG, JPEG or PNG format. The total size of your files must not exceed 45 MB. A single file may be a maximum of 15 MB. • For married couples or registered civil partnerships: marriage certificate/civil partnership certificate • Current Meldebescheinigungen (registration certificates) alternatively: online extracts from the register of residents and copies of your identity cards / ID cards / passports (front and back) • For married opposite-sex couples: approved treatment plan from the statutory health insurance fund(s), including the covering letter(s) from the health insurance fund(s) If the cost estimate on the treatment plan is not filled in, a separate cost estimate from the Kinderwunschzentrum (fertility clinic) must be submitted • A cost estimate from the Kinderwunschzentrum (fertility clinic), if the costs are not stated on the treatment plan • Medical certificate completed by the fertility clinic for the application for funding under assisted reproduction (künstliche Befruchtung) (under "Formulare" [forms]) • Medical certificate for married couples/couples using donor sperm • Medical certificate for married couples/couples • Medical certificate for same-sex married couples/couples • Proof of additional benefits / bonus payments / cost coverage / rejection notices • If you have statutory health insurance and your health insurance fund grants additional benefits/bonus payments, please provide corresponding proof of this. • If you have private health insurance, please provide your private health insurance company's decision regarding your fertility treatment (cost coverage / rejection notice). Please also enclose the insurance terms and conditions relating to fertility treatment. • If you are entitled to Beihilfe (state allowance for civil servants), please provide your Beihilfe decision regarding your fertility treatment (cost coverage / rejection notice). • Step 3: Application for payment of benefits for Assisted Reproduction only possible online • The payment application must be submitted to LAGeSo within 2 months of receiving the last invoice / the last pharmacy receipt. • Please have all required documents and evidence ready for upload in PDF, JPG, JPEG or PNG format.
