• You have completed the prescribed course of study in psychotherapy and passed the state examination in the state of Berlin. • You have not been guilty of any conduct indicating that you are unworthy or unreliable to practice the profession. • You are fit, in terms of health, to practice the profession.

• Application for the granting of Approbation (full license to practice) as a psychotherapist Please submit a written application by post. • Proof of identity • If applicable, a certificate of name change if the name has changed since the certificate of the state examination was issued (for example, a marriage certificate) • Curriculum vitae in tabular form • Official certificate of good conduct (Führungszeugnis) service.berlin.de/dienstleistung/120926/ It must be no more than one month old at the time the application is submitted • Declaration stating whether criminal proceedings, or an investigation by the public prosecutor's office or professional disciplinary proceedings, are pending • Medical certificate confirming fitness to practice the profession of psychotherapist The certificate must be no more than one month old at the time the application is submitted • Doctoral degree certificate (if available)
